Unsatisfied requirements can cause financial aid delays, prevent students from receiving aid offers, and prevent aid from disbursing. Students are notified via their MIX email on their WVU Portal if they have unsatisfied requirements.
Students can view everything online. Please note that in addition to unsatisfied requirements available online, aid programs may have enrollment requirements that must be met for the aid to disburse.
If one of your requirements is "Unsatisfied (Outstanding) Task(s)" you will be directed to the StudentForms website to complete these tasks. You will log in with your WVU username and password. If this is your first time, you must verify your first/last name, date of birth, and social security number. More information is available on the StudentForms Account webpage.
** Please note if you have a scholarship or private loan check made out to both you and the University, you must endorse the check before we can apply it to your student account. You will be contacted via your MIX email should we receive a check that needs endorsed by you.
The U.S. Department of Education requires schools participating in federal student aid programs to verify information submitted on the Free Application for Federal Student Aid (FAFSA) for students selected for verification. Verification involves the review of tax documents and family size information for accuracy. To complete verification, students and parents, as applicable, may be asked to submit federal tax documentation, forms, and/or other additional documentation. Students may see "Unsatisfied (Outstanding) Task(s)" on their WVU Portal under their unsatisfied requirements to indicate additional information is needed for their financial aid.
Students may also want to request a Dependency Appeal or SAI Calculation Appeal (also known as a special circumstance or financial hardship).
To satisfy the requirement or make these requests, you must visit the StudentForms website. If you have not set up your account, you will be prompted to do so. Once the account is set up, you will be able to access the website with your usual WVU username and password.
Please note that sometimes the signature of a parent is needed on a form. If so, after you have e-signed your form, you will be presented with the steps to send an e-signature link to your parent via email.
Some of the information in the register account screen is pre-populated. You may need to update the information to match your FAFSA exactly for the first and last name. If the information in the fields does not match your FAFSA or if there is not a FAFSA on file, you will receive an error message when trying to set up your account.

To create a parent account after receiving the e-signature request from the student:
Once your account is created, you can review and e-sign the student's form. You can also say the form is not approved. If not approved, the student can make corrections, e-sign, and send a new e-signature request to the parent. If the parent chooses to opt out of e-sign, both the student and parent must physically sign the form. Review Cannot E-sign section for instructions.
Important: After you create your account, you will be sent an email to verify your email address. You must verify your email address within 24 hours of receiving the email. If your email address is not verified, you will not be able to reset your password in the future without contacting the WVU Hub.

If you (or your parent) are unable or do not want to use the e-signature option, you (or your parent) can opt out. Please note that if the parent chooses to opt out of e-sign, both the student and parent must physically sign the form. Students may also choose to opt back into e-sign at any time.
To submit the form with physical signatures instead of using the e-signature:
Prior to selecting the "Finish" button on your tasks, you may select the "Edit" button to edit documents on your account on the StudentForms website. If you sent an e-signature request to your parent for the form, you must cancel that e-signature request first by clicking the "Cancel Request" button next to the e-signature request on your task list.
If the task is a web form (a form you fill out on the website), you would just need to click the "Fill Out" button next to the task to make corrections. You will go through the signature process again after making corrections.
Please note that if you have selected the "Finish" button to submit all your tasks for review, you must contact the WVU Hub to request that we reject the document(s), so you will have the option to make edits again.
Students should only be locked out of their StudentForms account if they are locked out of their WVU login. Visit the WVU Login Self-Service website to reset your account.
Parents are locked out of their StudentForms account after five unsuccessful login attempts. You must contact the WVU Hub to unlock your account. You can submit an online request (choose "Request Assistance Now" then choose "Public Inquiry Submission"). You can also give us a call at 304-293-1988 during regular business hours.
