Unsatisfied Requirements

General Information

Unsatisfied requirements can cause financial aid delays, prevent students from receiving aid offers, and prevent aid from disbursing. Students are notified via their MIX email on their WVU Portal if they have unsatisfied requirements.

Reviewing My Unsatisfied Requirements

Students can view everything online. Please note that in addition to unsatisfied requirements available online, aid programs may have enrollment requirements that must be met for the aid to disburse.

  1. Go to your WVU Portal
  2. Enter your login information ( WVU username and password)
  3. Click on the "Financial Aid Information" button
  4. Select the appropriate award year
  5. Review unsatisfied requirements on the Home page

If one of your requirements is "Unsatisfied (Outstanding) Task(s)" you will be directed to the StudentForms website to complete these tasks. You will log in with your WVU username and password. If this is your first time, you must verify your first/last name, date of birth, and social security number. More information is available on the StudentForms Account webpage.

** Please note if you have a scholarship or private loan check made out to both you and the University, you must endorse the check before we can apply it to your student account. You will be contacted via your MIX email should we receive a check that needs endorsed by you.

Verification

The U.S. Department of Education requires schools participating in federal student aid programs to verify information submitted on the Free Application for Federal Student Aid (FAFSA) for students selected for verification. Verification involves the review of tax documents and family size information for accuracy. To complete verification, students and parents, as applicable, may be asked to submit federal tax documentation, forms, and/or other additional documentation. Potential Financial Aid Payment Issues

Students and families should review the list below to determine why aid did not disburse (pay towards their balance due):

  • Submit a Free Application for Federal Student Aid (FAFSA) for this academic year
    • The FAFSA must be completed and submitted on the Federal Student Aid website every academic year you are interested in financial aid. Visit our Applying for Aid webpage for more information.
    • Make sure the FAFSA was for the correct academic year. For example, Fall 2025, Spring 2026, and Summer 2026 are under the 2025-2026 FAFSA.
  • Review and complete unsatisfied requirements
  • Accept your financial aid
  • First-time loan borrowers:
    • There are additional requirements that must be completed depending on the type of loan. Visit our Loans webpage for more information about each fund type and their requirements.
    • Students interested in additional loan options may also apply for a federal PLUS loan or a private loan. Visit our Loans webpage for more information.
  • Continuing student loan borrowers:
    • PLUS and private loans are credit-based loans which require an application each year the student wishes to utilize those funds. Visit our Loans webpage for more information on application or additional requirements for disbursement based on loan type.
  • Private loan or private scholarship check that needs endorsed
    • If you have a scholarship or private loan check made out to both you and the University, you must endorse the check before we can apply it as payment. You will be contacted via your MIX email should we receive a check that needs endorsed by you.
  • Maintaining satisfactory academic progress standards for financial aid eligibility
  • Meeting enrollment requirements
  • Repeating courses this semester
    • Previously passed courses can only be repeated one time and still be eligible for aid and count toward your enrollment requirements for aid. Students repeating previously passed courses more than once may experience reduced eligibility for aid. Visit our Enrollment Standards for Financial Aid webpage for more information.

StudentForms Account

Students may see "Unsatisfied (Outstanding) Task(s)" on their WVU Portal under their unsatisfied requirements to indicate additional information is needed for their financial aid.

To satisfy the requirement or make these requests, you must visit the StudentForms website. If you have not set up your account, you will be prompted to do so. Once the account is set up, you will be able to access the website with your usual WVU username and password.

Please note that sometimes the signature of a parent is needed on a form. If so, after you have e-signed your form, you will be presented with the steps to send an e-signature link to your parent via email.

Setup Instructions

Setting Up My StudentForms Account

  1. You must submit a Free Application for Federal Student Aid (FAFSA) to the WVU System (school code 003827 for all campuses). If you do not have a FAFSA on file, you cannot set up/link up your student account.
  2. Go to the StudentForms website
  3. Enter your WVU username and password. If you are already logged into a WVU platform like the Portal, it may not ask you to do this.
  4. The first time you log in, it will ask you to fill out some information to link up your WVU student account to your FAFSA information. It will not ask you this for future log ins. Fill out your:
    1. First name (as listed on your FAFSA)
    2. Last name (as listed on your FAFSA)
    3. Date of birth (as listed on your FAFSA)
    4. Social Security number (as listed on your FAFSA)
    5. Preferred email address (and confirm the email address)
    6. Phone number if you would like to receive text messages for tasks you need to do (tasks that could prevent your financial aid offer and/or disbursement)
  5. Click on "Register Account"

Some of the information in the register account screen is pre-populated. You may need to update the information to match your FAFSA exactly for the first and last name. If the information in the fields does not match your FAFSA or if there is not a FAFSA on file, you will receive an error message when trying to set up your account.

Setting Up My Parent StudentForms Account

Sometimes the signature of a parent is needed on the StudentForms task. If so, after the student has e-signed their form, they will be presented with the steps to send an e-signature link to the parent via email.

Please note:

  • If a parent did not pass the Social Security Administration check through the FAFSA (matching name, date of birth, and Social Security number), that parent cannot e-sign forms. If at least one parent did not pass the check, the student is automatically opted out of e-sign. Review Cannot E-sign section for instructions.
  • Parents cannot create an account or view documents for e-signature until the student sends the e-signature request. Only the parent the student sends the request to will be able to set up an account.

To create a parent account after receiving the e-signature request from the student:

  1. Click on the link in the e-signature request email from the student.
  2. Click on the "Create Account" button. Parents can only create one account per student. If they have multiple students, a separate account will need created for each student if they need to e-sign documents for each.
  3. Fill out the requested information (email address, username, and password). Please note that if you are a parent who must set up multiple accounts for different students, you must use a different username and email address for each account.
  4. Confirm your parental information from the FAFSA (your first name, last name, date of birth, and Social Security number). This information must match the student's FAFSA exactly for you to set up your account.
  5. Confirm your student's information from the FAFSA (their first name, last name, date of birth, and Social Security number). This information must also match the FAFSA exactly.

Once your account is created, you can review and e-sign the student's form. You can also say the form is not approved. If not approved, the student can make corrections, e-sign, and send a new e-signature request to the parent. If the parent chooses to opt out of e-sign, both the student and parent must physically sign the form. Review  Cannot E-sign section for instructions.

Important: After you create your account, you will be sent an email to verify your email address. You must verify your email address within 24 hours of receiving the email. If your email address is not verified, you will not be able to reset your password in the future without contacting the WVU Hub.

Sending an E-Signature Request

Sometimes the signature of a parent is needed on the StudentForms task. After you e-sign your form, you must:

  • Select which parent you would like to sign. You will only see the parent(s) listed on the FAFSA as an option. Parents must have passed the Social Security Administration check on the FAFSA to be eligible to e-sign. If at least one parent did not pass the check, you are automatically opted out of e-sign. Review  Cannot E-sign section for instructions.
  • Enter your parent's email address... and give your parent a heads up that this is coming. The email you list must be different than your email address.
  • Confirm your parent's email address.
  • Click the "Send Request" button.
  • Once the request has been sent to your parent, you are not able to edit your form unless you cancel the request. You can cancel or edit your request by using the "Cancel Request" or "Edit Request" buttons next to the e-signature request line on your StudentForms tasks.

Troubleshooting

Cannot E-sign

If you (or your parent) are unable or do not want to use the e-signature option, you (or your parent) can opt out. Please note that if the parent chooses to opt out of e-sign, both the student and parent must physically sign the form. Students may also choose to opt back into e-sign at any time.

To submit the form with physical signatures instead of using the e-signature:

  • On the review and sign page of the webform, you can choose to opt-out of e-sign.
  • Once the e-sign option is no longer available, you (the student) will be presented with instructions. You must click the "Confirm" button to move forward.
  • After you confirm, click on the "Download" button to download a copy of the form.
  • Print and sign the document. If a parent signature is required, have your parent physically sign the form as well.
  • Once the form has been printed, signed, and dated, scan or take a picture of your form. The file must be saved in a PDF, PNG, BMP, or JPG format to upload.
  • Return to your task list on your StudentForms account.
  • Click on the upload button to upload your document(s). Please make sure you upload all pages.

Editing My Form

Prior to selecting the "Finish" button on your tasks, you may select the "Edit" button to edit documents on your account on the StudentForms website. If you sent an e-signature request to your parent for the form, you must cancel that e-signature request first by clicking the "Cancel Request" button next to the e-signature request on your task list.

If the task is a web form (a form you fill out on the website), you would just need to click the "Fill Out" button next to the task to make corrections. You will go through the signature process again after making corrections.

Please note that if you have selected the "Finish" button to submit all your tasks for review, you must contact the WVU Hub to request that we reject the document(s), so you will have the option to make edits again.

Locked Out of My Account

Students should only be locked out of their StudentForms account if they are locked out of their WVU login. Visit the WVU Login Self-Service website to reset your account.

Parents are locked out of their StudentForms account after five unsuccessful login attempts. You must contact the WVU Hub to unlock your account. You can submit an online request (choose "Request Assistance Now" then choose "Public Inquiry Submission"). You can also give us a call at 304-293-1988 during regular business hours.
